Children and Teen Forms


  • Thank you for your desire to work with our children and teens! This application is to be completed by all applicants, volunteer or compensated, involved in the supervision or custody of minors. This form is being used to help the Church provide a safe and secure environment for our children within our facilities.  


    This completed form can be dropped off to the Church Office or emailed to worship@rockfordbaptist.org.


    Before filling out the application, we ask that you please read our abuse prevention policy and children's worker guidelines.

    • Due to liability, anyone who works with children / teens is REQUIRED to complete this application before being permitted to serve.
    • Upon completion of this application it will be reviewed by the CE Committee or Elders for approval.

  • This form is REQUIRED to be on file for all RBC KIDS & must be updated yearly. 

    • You may type in the information & then print to sign. It can be then mailed, dropped off at the church office or email back to

  • This form is REQUIRED to be on file for all middle and high school students and must be updated yearly.

    • Print and fill out. It can be then mailed, dropped off at the church office, or scanned and emailed to worship@rockfordbaptist.org.
